    My recommendation is to get Leigh White's book Dita for Print. She has an example to do exactly that and it works perfectly. It's an excellent book well worth the $ and time investment.

    Regarding the attribute sets for headers that don't seem to be used, you could try to create separate attribute files and importing them into the custom.xsl file, e.g.,:
    ….
     

       
     
    ….

    And this is inside my OPMANUAL_headers.xsl file:

            20pt
          solid
            orange

    Have you tried to modify the template that processes topics to add a when statement that says “if this is a level 2, use topicLevel2 attributes” and then create an attribute set for “topicLevel2” that includes a pagebreak?

    In the when statement, you could  use a depth variable like this (the depth variable idea comes from the topic numbering template that you can get from http://dita-generator-hrd.appspot.com/pdf-plugin/)

    You can try using the outputclass attribute on the stentry element and  modifying the template that starts with to change the orientation.

    •attr-set-reflection.xsl now actually works.  It used to be that some attribute sets were “fakes” – topic.topic.title for example couldn't have or in it.  Now all attributes sets are “real” – they all support normal xsl behavior

    Yes – I tried the same thing in the “….DITA_OTpluginsxmfoCustomizationfoattrsxm-cfgcommons-attr.xsl” file and saw the same behavior I described above.

    I just tried other attribute sets this morning:

    I get the “bad attribute” error when I use a “choose” statement with the topic.title and topic.topic.title attributes.
    I do not get the error when I use a choose statement with the topic, p, or fig.title attributes.

    What worked for me so far (not extensively tested yet and I don't know if this is what you were looking for) is the RendexX extension http://services.renderx.com/Content/support/xep/tutorial.html

    I made the template change suggested by Derek. It works well for me except that the placement attribute value is ignored when it is set at inline. All my images have a break whether I want one or not.  I tried a few things wiht the template but could not resolve this issue. Any thoughts? Thanks. Isabelle
