When customizing the headers for PDF output, all changes for odd and even headers take effect, but param __body__first__header is ignored. Is there ia reason this is ignored? Also is there a param to distinguish the header for the first page of each topic from the rest of the body?
Any enlightenment is greatly appreciated.
