There is no simple way to do this (a checkbox or special setting). The DITA Open Toolkit is not designed to support this so you would need to make modifications to the XSLT that ultimately generates the HTML. Those files live here (with a default installation):
C:Program FilesCommon FilesXMetaL SharedDITA_OTxslxslhtml

…and your changes would ultimately need to override logic in this file: dita2htmlImpl.xsl

Injecting something like the following into each of the XSLT templates (starting at about line 646 which begins with ) may be good enough for you:

It does not give you lots of flexibility, but it should give you the HTML you need.
