Does anyone know the purpose of file C:\Users\[username]\AppData\Roaming\SoftQuad\XMetaL Shared\DITA_OT\demo\xmfo\Customization\fo\xsl\xm_title_number_fix_v2.xsl? It looks like it handles heading numbering but I'm not sure it's actually used anywhere.

This is correct. The files xm_title_number_fix.xsl and xm_title_number_fix_v2.xsl are not referenced by any other part of the system and are not used.
